Although Medical Necessity is the ultimate driver of an E&M service, it's the medical record documentation that determines whether an E&M code accurately represents the care provided. It is estimated that over 80 percent of primary care services are E&M encounters. Increased scrutiny by all payers has resulted in the need to take a closer look at our documentation. WPS Medicare Part B has provided us with some of the most common E&M documentation errors they see during the medical review process. During this interactive session we will use both the 1995 and 1997 documentation guidelines to examine these common errors and provide solutions ensuring your documentation is rock solid.
