Access Community Health Centers, Madison
Adolescent Health Education-Year 2 Expansion
Year 2 of a project with Lussier Community Education Center to provide health education to middle and high school youth in a targeted area of the Madison Metropolitan School District plus an expansion to reach more youth by partnering with the East Madison Community Center and Centro Hispano.

Healthy Classrooms Foundation, Statewide
2011 Healthy Classrooms Symposium
A symposium open to all members of the community focusing on the importance and implementation of public health into a K-12 classroom setting.

Prairie du Chien Historical Society, Inc./Fort Crawford Medical Museum
Swift United States Army Hospital Exhibit Project
A new museum exhibit to tell the story of Swift Hospital, the only on of three Wisconsin Civil War Hospitals still standing. This project will expand the public’s understanding of how Civil War wounded were treated and cared for in stark contrast to present day care.
Award: $4,100 (Funded by the Earl and Alice Thayer Fund)
Public Health Madison & Dane County
Tuberculosis Home Sputum Collection: A Step-by-Step Guide
An educational DVD that teaches clients how to effectively collect and handle sputum specimens in the home for the diagnosis and treatment of tuberculosis in order to overcome language and general literacy barriers.

Ripon Medical Center, Ripon area
healthYouth Program
A pediatric obesity program at Ripon Medical Center geared toward overweight and obese patients in the region. healthYouth is not a weight loss focused program, but rather a lifestyle modification and behavior change program.
Award: $5,000
Sauk Prairie Memorial Hospital & Clinics, Sauk County
Lifestyle Change for Rural People Suffering from Obesity and Chronic Illness
Increase access for the rural population of Sauk County to a pilot-tested lifestyle change program that improves health outcomes for patients suffering from obesity and poorly controlled chronic conditions such as diabetes or asthma.
Award: $12,000 (Funded in part by the PIC Wisconsin Fund and the Joss Trust Fund held by the Wisconsin Medical Society Foundation)
