Immunizations Required by Age/Grade Level


2008-2009 School Year and Following School Years Requirements


The following chart displays the minimum basic and booster immunizations required under Wisconsin Statute ยง 252.04(2). It does not, however, represent all the recommended immunizations for those individuals who begin immunizations in infancy and follow currently accepted immunization schedules.

1 For kindergarten only, at least one dose to be received after 4 years of age unless medically contraindicated. A dose received 4 days or less before the fourth birthday is acceptable.
2 A single dose, booster immunization against tetanus, diphtheria and pertussis is required on entrance to grades 6, 9 and 12, beginning with the 2008-09 school year. See Wis. Admin. Code HFS 114 sub. (3) for phase-in of other grades.
3 Two doses of Var vaccine are required on entrance to grades K, 6 and 12, beginning with the 2008-09 school year. See Wis. Admin. Code HFS 114 sub. (3m) for phase-in of other grades.
4 At least one dose to be received after 12 months of age unless medically contraindicated. A dose received 4 days or less before the first birthday is acceptable.
5 Required on entrance to a day care center, beginning with the 2008-09 school year.
