Thank you for your interest in applying to the Wisconsin Medical Society Foundation Community Grant Program.
In addition to the online application, you will be asked to upload the following documents:
- Program expense and revenue spreadsheet that include those expenses and revenues that will be allocated to the proposed project.
- Physician letter of support (preferential consideration is given to applications that include a letter of support from a Wisconsin Medical Society physician member).
- Signed Certification.
- Any additional supporting documents you would like the Grant Committee to consider.
