MADISON (Oct. 23, 2014) — Honoring Choices Wisconsin (HCW), the Wisconsin Medical Society’s advance care planning initiative, is pleased to announce six new participating organizations. Mercy Health System in Janesville, Reedsburg Area Medical Center, Bellin Health in Green Bay, Westfields Hospital & Clinic in New Richmond, Hudson Hospital & Clinic and Amery Hospital & Clinic all have signed on to build systems and workflows to offer advance care planning conversations to their patients. The new participants will begin training this year and launch trial implementations in 2015.
“Advance care planning conversations make an enormous difference in the lives of patients, their family and friends, and the team that cares for them, especially in the area of end-of-life care—something we all must deal with,” said Society President Richard A. Dart, MD. “We’re thrilled to have these six high-quality organizations joining the advance care conversation.”
The new groups were announced during yesterday’s Sharing the Experience Conference held in Madison. Since HCW launched in September 2012, more than 3,000 patients have engaged in conversations about their future medical decisions, including end-of-life preferences, with HCW-trained facilitators. These conversations help patients understand their health care treatment options, appoint a health care agent, clarify goals, weigh options about the kind of care and treatment they would want or not want and communicate those wishes to family, friends, clergy, physicians and others.
Through HCW, the Society serves as a convener, coordinator and catalyst to build clinical pilot projects based on proven concepts, methodologies, training systems and materials, combined with outreach in communities across the state. The six new groups join HCW’s 16 existing participants, aiming to make advance care planning a routine and standard part of care.
