Sharing the Experience Conference

Representing 22 health care organizations across the state, Honoring Choices Wisconsin (HCW) stakeholders gathered in Madison October 22 for the 2nd annual Sharing the Experience Conference. The event showcased the two-year growth and reach of HCW and provided a platform for stakeholders to share their successes and offer help to their peers as they build advance care planning systems.
HCW also announced six new participating organizations and unveiled its first video, It’s About the Conversation, produced in collaboration with Wisconsin Public Television. The video is the first in a proposed six-part series to prepare patients, physicians, health care teams and the public to have advance care planning conversations.
Be sure to download the Conference program, which shares the experiences and insights of many Honoring Choices Wisconsin participating organizations on their trial implementations, successes, challenges and lessons they have learned along the way. The program presents Honoring Choices Wisconsin’s many accomplishments this year, as well as its plans for the future.
