Advance Directive
An advance directive is document in which a person states goals, values and beliefs about health care treatment decisions, including who should make those decisions, in the event that person can no longer make those decisions for him/herself.
PLEASE NOTE:
It is highly recommended that the Honoring Choices Wisconsin (HCW) advance directive is completed with the assistance of a certified advance care planning facilitator for accurate completion, valid witnessing, and the reliable filing of your document.
