Improve the health of the people of Wisconsin by supporting and strengthening physicians' ability to practice high-quality patient care in a changing environment.

Jan. 31 is deadline for filing electronic medical records credit requests

Physicians and other health care professionals who qualify for the Wisconsin Electronic Medical Records Credit must submit their certification requests to the Department of Revenue (DOR) by January 31. Because of the limited processing time, DOR strongly recommends electronic filing of the certification request (Schedule EM-C).

All requests must include the qualified hardware purchased as well as the certified health information technology list (CHPL) number from the Office of the National Coordination for Health Information Technology. Schedule EM-C will be available on the DOR website from January 3 to January 31, 2013. More information, including answers to frequently asked questions, also is available online.

The Wisconsin Electronic Medical Records Credit is available to certain individuals and entities for the amounts paid for information technology hardware and software used to maintain medical records in electronic format. A Fact Sheet about the credit is on the DOR website.
