List of patients moving off Medicaid may be requested online

Physicians can visit the ForwardHealth Provider Portal to request a list of their patients who are transitioning from Medicaid to the Health Insurance Marketplace. This allows physicians and their staffs to proactively reach out to their patients who may be affected by the change.

Lists can be requested by individual National Provider Identifier only, which means health systems cannot access a list for all of their physicians. Instructions are provided after logging in to the Provider Portal.

Wisconsin’s 2013-2015 state budget moves approximately 85,000 parents and caretakers who earn between 100 percent and 200 percent of the federal poverty level (FPL) from BadgerCare (Medicaid) rather than expand eligibility through the Affordable Care Act (ACA). The FPL for childless adults in Wisconsin is $11,490.
