Register today for 2015 Sharing the Experience Conference

Registration is now open for Honoring Choices Wisconsin’s (HCW) third annual Sharing the Experience Conference, Thursday, Oct. 22, at the Alliant Energy Center in Madison.

Through HCW, the Wisconsin Medical Society serves as a convener, coordinator and catalyst to build clinical pilot projects based on proven concepts, methodologies, training systems and materials, combined with outreach in communities across the state. There are currently 26 health care organizations statewide participating in the initiative.

The Sharing the Experience Conference is an opportunity for participants to share successes, challenges and lessons learned, while striving to promote and improve advance care planning across the state. Keynote speakers include Tracy Schroepfer, PhD, MSW, MA, associate professor at the University of Wisconsin-Madison School of Social Work and Linda Briggs, MS, MA, RN, associate director of Respecting Choices®.

The event also will feature breakout sessions for facilitators, team leaders, physicians, people new to this initiative and those whose advance care planning efforts are expanding. Three advance care planning champions will be recognized for their extraordinary work as well.
